Delva
Tool and Machine Corporation currently offers our
employees two “voluntary” Dental plans.
The Dental plan is paid by you the employee with the
cost of the coverage being deducted from your paycheck.
The
first Dental plan available is called Horizon Dental
Choice (HDC). This is the more expensive choice which
may help you understand which plan you have. You may
also refer to your Horizon Blue Cross & Blue Shield
Dental ID card and look for “HDC” or “Horizon
Dental Choice”.
The
key to HDC is that care must be coordinated through
the participating HDC dentist that you select to use
as your “primary dentist”. Remember, this
plan only includes dentists in New Jersey! This plan
will cover 100% of all eligible Preventive and Basic
services with no co-payments, no maximums or deductibles
when receiving those services from your primary HDC
dentist! This plan will also cover 50% of charges
for all eligible major and specialty services.

Dental Choice Plan
The
second Dental plan available is called Horizon Dental
PPO Access. This is the less expensive choice which
may help you understand which plan you have. You may
also refer to your Horizon Blue Cross & Blue Shield
Dental ID card and look for “PPO Access”.
The
key to the PPO Access plan is that it works based
on a savings schedule! When you receive treatment
form one of the dentists in the Horizon Dental PPO
Network, (includes New Jersey, New York & Pennsylvania
Dentists) your costs are “reduced” significantly.
