Tool and Machine Corporation currently offers
our employees a choice from two Medical Insurance
Plans. Both plans are underwritten by Horizon
Blue Cross & Blue Shield of New Jersey. Simply
“click on” the plan you are enrolled
in to learn more!
The
first plan we offer is called Direct Access
Plan EM. This is the more expensive choice
that our employees were given during open enrollment.
This may help you understand which plan you
have. You may also refer to your Horizon Blue
Cross & Blue Shield ID card and look for
“Direct Access”.
The
feature of this plan is the direct access to
participating Doctors and Specialists without
the need to obtain a referral! While you
are encouraged to select a primary care physician,
it is not necessary for you to obtain a referral
from the primary care physician before visiting
a specialist. This plan will also provide you
with the freedom to seek “out of network”
care from a “non-participating” provider.
If you render services from a Doctor or Hospital
that does not participate with Horizon BCBS,
you will be subject to your Deductible and Co-Insurance.

The
second plan we offer is called Point of Service
(POS) Design AM. This is the less expensive
choice that our employees were given during
open enrollment. This may help you understand
which plan you have. You may also refer to your
Horizon Blue Cross & Blue Shield ID care
and look for “POS”.
The
feature of this plan is that you must select
a Primary Care Physician (PCP). You must
also obtain a referral from your Primary Care
Physician in order to visit a specialist. This
plan will also provide you with the freedom
to seek “out of network” care from
a “non-participating” provider. If
you render services from a Doctor or Hospital
that does not participate with Horiozn BCBS,
you will be subject to your Deductible and Co-Insurance.
